Compiling a periodontal chart

Periodontitis is an inflammation of the periodontium — the tissues surrounding the root of the tooth. It is a very common and insidious disease, as it often develops without noticeable symptoms or with only mild signs. However, it is periodontitis — not tooth decay — that is the leading cause of tooth loss in adults.

The periodontal chart is a medical document that records all negative changes in the periodontal tissues based on the results of a comprehensive diagnostic examination. Treatment planning for periodontitis begins with the examination and creation of this chart.

What the Periodontal Chart Includes
  • Description of the condition of the gums and bone tissue around each tooth
  • Assessment and location of plaque on each tooth, including both visible and subgingival areas
  • Evaluation of gum bleeding levels
  • Description and location of purulent pockets
  • Oral hygiene recommendations
How the Periodontal Chart Is Created
  • Stage One: Photo documentation. A periodontist takes detailed, close-up photographs of the oral cavity to visually assess the condition of each tooth and later compare the initial images with those taken after treatment.
  • Stage Two: If needed, a CT scan or X-ray examination is performed.
  • Stage Three: Probing. The depth of all periodontal pockets is measured using a special probe. The results and calculated indices are recorded in the periodontal chart.
Why the Periodontal Chart Is Important
  • Initial diagnosis: The periodontal chart serves as the starting point for developing an individualized periodontitis treatment plan, as well as for selecting hygiene products and oral care recommendations.
  • Treatment monitoring: Comparing the initial periodontal chart with follow-up charts during treatment clearly reflects changes and helps assess the effectiveness of interventions.
  • Long-term monitoring: Comparing charts from routine checkups over time allows for early detection of periodontal condition changes and potential deterioration at a stage when it can still be easily managed.
